Getting started

  1. Install Mint Forms from the Shopify App Store. The app home shows a short setup checklist.
  2. Build your form: click Create a form and pick a template (contact, wholesale registration, quote request or blank). Add, edit and reorder fields; the preview on the right runs the real form.
  3. Add it to your store: on the app home choose where (contact page, a page template, home page or product pages) and click Add form to store. The theme editor opens with the Form block from Mint Forms added; move it if you like and click Save. The block shows your first form; to show another form, paste its ID (on the Forms page) into the block's Form ID setting.
  4. Check notifications: click Send test email on the app home. New-submission emails go to your store's contact email unless you add other recipients in a form's Notifications section.

How submissions work

  • The form checks answers in the shopper's browser (with messages linked to each field for screen readers), then our server checks them again against your saved form before anything is stored.
  • Each submission is saved in the Inbox first. Then, if the form has notifications on, it's emailed to you with the shopper's email as the reply-to address. If the email can't be sent, the submission shows "Email waiting" and is retried for 3 days.
  • Spam protection: a hidden trap field, a minimum fill time and limits on how often one connection or one store can send.
  • Submissions and files are deleted automatically after your retention period (Settings; 90 days by default). Export a CSV first if you need to keep them longer.
  • Each successful submission fires a mintforms:submitted event on the page (with the form ID), which you can use for your own analytics.

Common questions

The block says "no form with the ID …". Copy the ID from the Forms page (8 letters and numbers), or leave the Form ID setting empty to show your first form.

The form says it isn't accepting responses. On Free, that happens once the month's 100 submissions are used; it reopens on the 1st of next month (UTC) or as soon as you upgrade. It also happens when a form is turned off, or when a second form is used on Free (one form is live on Free).

I didn't get an email. Open the submission in the Inbox: it shows whether the email was sent, is waiting or failed, and why. You can send it again from there. Check your spam folder for emails from mail.stickermint.com.

A customer wasn't created (Pro). The submission shows Shopify's reason (for example, a phone number already used by another customer) and a Try again button. Customers are matched by email address, so the form needs an Email field.

I changed the app proxy URL. The storefront form sends to /apps/mint-forms. If you changed that path under Settings → Apps → Mint Forms → App proxy, change it back.

Billing

Free includes one form and 100 submissions per month, with the inbox, CSV export, notifications and spam protection. Pro is billed through Shopify at $4.99/month after a 7-day free trial and adds unlimited forms and submissions, file uploads, conditional fields and creating or updating customers. Cancel anytime from the app's Plans page or by uninstalling.

Privacy

See our privacy policy, security policy and data processing addendum. In short: only you can see submissions, they're deleted on the schedule you set, and privacy requests from Shopify are handled automatically.
