Build the form, place it with the theme editor, and answer shoppers from your inbox or your email.
Short and long text, email, phone, number, dropdown, radio buttons, checkboxes, date, hidden values, a marketing-consent box and text blocks. Required fields, length and number limits, date ranges, two or three fields per row, and a live preview of the real form.
Search, filter by form, mark read or unread, delete, and export to CSV — on the free plan too. Each submission shows the questions as the shopper saw them and whether its email notification was delivered.
You get an email for each submission; replying answers the shopper. The submission is stored before any email is sent, so if email can't go out it's still in your inbox, marked as waiting, and retried for 3 days.
A hidden trap field, a minimum fill time with a signed token, and rate limits per connection and per store. Every answer is checked against your saved form on the server, so a bot can't add fields or skip required ones.
Show a field only when an earlier answer matches, like an order number for questions about an order. Let shoppers attach images or PDFs up to 10 MB, checked by their content, stored privately and only downloadable from your admin.

Questions merchants ask
Do I need to edit my theme?
No. Mint Forms is an app block: one click opens the theme editor with the block ready to place on your contact page, home page, product pages or any page template. It uses your theme's fonts, colours and button style, and the form is part of the page's HTML, so nothing jumps around while it loads.
What happens after 100 submissions on the Free plan?
The form tells shoppers it isn't accepting responses until the 1st of next month (UTC), and the app shows you a notice. Messages are never silently dropped, and upgrading reopens the form right away. Nothing is charged automatically.
What if a notification email can't be sent?
The submission is already saved in your inbox. The app shows the email as waiting or failed, shows a notice on the app home, and retries sending automatically for 3 days. You can also resend it from the submission.
How is spam stopped?
A hidden field only bots fill in, a minimum time between starting and sending the form (checked on our server with a signed token), and limits on how often one connection and one store can send. Submissions are validated against your saved form, never against what the browser claims.
Can I use it in my language?
Yes. Every label, button, message and error text is yours to edit on each form.
What do you store and for how long?
The answers shoppers send and any files they attach, kept for the period you choose (90 days by default, from 1 to 730 days) and then deleted automatically. Privacy requests from Shopify (data export and erasure) find every submission of that shopper. Details are on our privacy page.
